The goal of this program is to improve identification and management of type2 diabetes in vulnerable populations and increase knowledge about how the impending shortage of primary care physicians will change management of patients with type2 diabetes. After hearing and assimilating this program, the clinician will better be able to: 1.Identify vulnerable groups of patients with type2 diabetes and explain why they are susceptible to disease-related complications. 2.Describe how improvements in self-efficacy can improve self-management in vulnerable patients with type2 diabetes. 3.Summarize key revisions to the American Diabetes Association standards of care for patients with type2 diabetes. 4.Describe how the shortage of primary care providers has shifted management of type2 diabetes to a team approach involving multiple clinicians.
