The goal of this program is to improve diabetes care by incorporating the strengths and expertise of nonphysician providers into primary care practices. After hearing and assimilating this program, the clinician will better be able to: 1.Summarize the benefits of diabetes self-management education (DSME) for improving physiologic and psychosocial outcomes. 2.Describe interventions to eliminate common barriers to participation in DSME. 3.Develop a diabetes education program that address the needs of patients with food insecurity. 4.Implement in-person and remote management strategies to improve efficiency of patient care in a nurse practitioner-based practice for diabetes.
